New Patient Referral Process
If you are a patient, physician's office, attorneys office, claims adjuster, urgent care, or nurse case manager, we REQUIRE the following information:
New Patient referrals will require the following: Authorization from the insurance carrier specifying the physician and role (PTP, Secondary Physician, etc), DOI and accepted body parts, any diagnostic testing (X-Ray, MRI’s), patient demographics, phone number and billing remit to address.
Reminder:
Patients are required to bring their MRI CD and any medical information that will assist the physician make the best treatment plan. Failure to bring their MRI CD, will result in a rescheduled appointment.
Attention Attorneys, Claims Adjusters for Claims older than 1 year or Patients who have had surgery with another physician:
Our physicians will review medical records prior to accepting your patient/client to make sure that they are the appropriate physician for their care. Our physicians subspecialize and therefore we require patients with multiple body parts to identify their number one complaint/concern so that we can schedule with the appropriate physician. Our physicians prefer patients who have had surgery, follow up with the original surgeon.
Future Medical Care:
If you are an establish patient and have not seen our physician within the last 6 months our office will require a new authorization from your claims adjuster prior to scheduling. If you have not treated with our physicians but have future medical care under your workers compensation claim our physicians will require a new authorization and review your medical records prior to scheduling.
Interpreter / Disability Accommodations:
Our office requires that any injured worker that requires disability assistance or accommodations make prior arrangements with their claim's adjusters, applicant attorney or employer. Failure to make the necessary arrangements will result in rescheduling the injured workers appointment.
Our office works with Southland Interpreters for Spanish interpreter only.
Therefore, if your injured workers need a Spanish interpreter for their evaluations, please issue an authorization to:
